Yes. All of Nikon and Canon’s flagship cameras do not come with a flash, such as the D3 series or the 1D series.
Olympus EP1 doesn’t come with a flash either.
Other than those very high end cameras which they’d expect you to buy dedicated external flash, only really cheap "toy cameras" will not come with any flash.
Im not a hundred percent sure if they do but I know you can buy digital cameras that have an option to use flash or not. It usually has a symbol of a lightening bolt with a circle and slash through it to let you know the flash is off.
Yes, but for most photographers having an on-camera flash is very handy. On the Nikons (and probably the Canons) the flash can be used to trigger off-camera flashes, which is VERY handy.
The top of the line Nikons do not have built-in flash but they are so sensitive to light that some of them can shoot quite well in almost near darkness, and they still have a "hot shoe" to put a flash on if you want one. The photographers that buy cameras like this probably already have professional flash equipment and don’t need an on-camera flash. These camera bodies cost $5,000 and more.
You can also just turn the flash off in the menus on most cameras.

Yes, most digital cameras have a flash button that can turn the flash off and to different flash settings.
